Community Nursing Team Leader - Reading
An exciting opportunity for a dynamic, strategic and enthusiastic individual to join our Community Nursing Team in Reading has arisen. CAMHS Community Rapid Response Team is a Berkshire wide service which provides urgent assessment and short-term interventions to young people (under 18 years) presenting in a mental health crisis. The service responds to young people who present to our acute hospitals and operates from 8am-10pm Monday to Friday and 9am-5pm at weekends.
As part of providing an urgent response to young people in crisis, the service aims to avoid/reduce Tier 4 admissions, delayed discharges and reduce frequent A&E attendance. The service provides comprehensive mental health assessments and clinical formulation to identify the young person’s needs as well as providing brief stabilising interventions including multi-agency consultation/liaison, as many young people present in crisis due to systemic issues such as family relational issues or school difficulties.
You will work in partnership with operational and professional leads across the service to ensure that we provide efficient, safe, responsive, consistent and equitable high quality assessments to children and young people and their families presenting in crisis.
As the successful candidate, you will be able to demonstrate that you are solution focused, resilient, and dynamic in your approach, able to embrace change and support others in doing so. You will need to have a CAMHS clinical or Social Work background and be able to demonstrate skills relevant to the role.
