Do you want to work as part of a dynamic and innovative mental health liaison service supporting the acute hospitals in Aintree University Hospital?

We are looking for experienced support worker who have a wide range of experience including older person’s mental health, CAMHS, substance misuse and adult mental health who can offer forward thinking and a dynamic attitude to ensure high quality service user care within a well established team.

The mental health liaison team provides support to service users presenting within the acute hospital trusts including A&E and inpatient wards covering the full hospital sites. The team complete assessments of mental health needs and provide treatment plans and signposting to relevant agencies. We also provide support, guidance and teaching opportunities to staff within the acute trust and work closely with all partner agencies.

The service covers 24 hours a day over 7 days a week and successful applicants will be expected to work over a 24 hour shift pattern including weekends, nights and bank holidays. There is an expectation that the successful candidate will be able to work across the entire Urgent Care pathway supporting the development of this exciting new service.

This position is situated in Fazakerley, closely linked to local accessible modes of transport for easy accessibility. Each service user has their own tenancy, and are supported to promote an independent and goal-oriented lifestyle. Ideal candidates will play a pivotal position in providing individualised support to service users in achieving and exceeding their goals within the framework of their own person centred plan.
Hours of work are: Up to 35 hours per week
Shift patterns: 08:00-22:00, days, nights, bank holidays and sleep-in duties
(according to the needs of the service).

Responsibilities:
* To develop and sustain warm and trusting relationships with service users.
* To maintain effective communication and liaison with service users, families and others whilst respecting appropriate confidentiality
* To encourage and support service users in expressing their needs, views and concerns. To enable
service users to make choices and decisions and to participate as fully as possible in planning and decision-making processes.
* To support service users in maintaining the safety, security and comfort of their homes.
* Enable service users to grow in confidence, competence and personal effectiveness in the following areas:
* Social skills/relationships
* Personal care & hygiene
* Daily living skills
* Using community resources and facilities
* Social, leisure and work activities
* Self organisation and coping abilities
* Personal safety
