A rare opportunity has become available to join the Dementia Rapid Response Team covering the Chesterfield, Bolsover and North East as a specialist support worker.

The Dementia Rapid Response team has been developed to support people to live in their own homes and to live well with dementia.
The team currently consists of a group of highly skilled and motivated individuals who provide rapid, evidence-based intervention to people who are experiencing an exacerbation of their illness and require intense input in order to prevent breakdown of care or hospital admission.

The team works 8am-8pm, Monday to Friday; 9am-5pm Saturdays, Sundays and bank holidays.
The post will suit someone with a proven record of working with people with dementia and the successful candidates will be expected to support the registered staff in delivering specialised, rapid and intensive input and treatment.

The successful candidate will be expected to cover a large geographical area so access to their own car is essential.

A rare opportunity has become available to join the Dementia Rapid Response Team covering the Chesterfield, Bolsover and North East as a specialist support worker.

The Dementia Rapid Response team has been developed to support people to live in their own homes and to live well with dementia.

The team currently consists of a group of highly skilled and motivated individuals who provide rapid, evidence-based intervention to people who are experiencing an exacerbation of their illness and require intense input in order to prevent breakdown of care or hospital admission.

The team works 8am-8pm, Monday to Friday; 9am-5pm Saturdays, Sundays and bank holidays.

The post will suit someone with a proven record of working with people with dementia and the successful candidates will be expected to support the registered staff in delivering specialised, rapid and intensive input and treatment.

The successful candidate will be expected to cover a large geographical area so access to their own car is essential.

DBS Check
The cost of undertaking a DBS (Disclosure and Barring Service) disclosure will be met by the individual. The Trust will meet the initial costs of the disclosure which will be deducted from your first month’s salary. The current price of a standard check is £23 and an enhanced check is £40.
