What we do
Clear Pathway care provide support for all adults aged 18 years and older of both sexes with the aim of supporting them back into the community. We promote individuals to keep in touch with their families and friends and be encouraged to maintain links with their own community where possible and to support everyone to integrate and become a valued member of their community.
Our services are tailored to each individual and specialise in Autism, Learning Disabilities, Communication problems, Challenging Behaviour and other associated health issues. We pride ourselves on providing a high standard of care and safety within a homely environment where the individual can feel valued, safe and take pride in their surroundings.
How we do it
A strength of Clear Pathway Care services is the training provided. Every member of staff, prior to commencing work completes a two-week induction which includes time set aside for observations of working practice. We support staff with ongoing professional development and QCF level 2,3 and 5 accreditation's. All staff members receive regular updated training on key training programmes e.g Child and Adult safeguarding and Fire training.
When staff members initially start employment, they are on a 6-month probationary period whereby there is a requirement to complete The Care Certificate, Autism Workbook and attend all mandatory training that has been provided. During this period staff will have monthly probation meetings and if successfully passing the probation period staff members will then receive bi-monthly supervision as a minimum and yearly appraisal.
The Role
We have vacancies for full-time Support Workers, experience is preferred but not essential. What we do need is caring, confident, and enthusiastic people who want to work with us to improve the quality of life for our service users.

A great opportunity has arisen to join our award winning Vulnerable Adult Support Team. We are looking for a highly motivated individual who will alongside our clinical team to support patients who attend the Emergency Department (ED) and Acute Medical Unit (AMU) in crisis, due to issues such as domestic and sexual abuse, substance use and homelessness.
You will join our dedicated Vulnerable Adult Support Team whose members are passionate about helping patients with complex issues such as domestic and sexual abuse, substance use and homelessness. The team is one of a kind, offering a unique opportunity to work alongside expert clinicians to provide person-centred interventions in the emergency care and acute medicine settings. The work of this team has been described as outstanding in two CQC inspections of the Trust (in 2015 and 2019) and the team also won the Emergency and Critical Care category in the 2018 Nursing Times Awards.

You will receive referrals from ED and AMU staff and will have the opportunity to spend time talking with each patient, so you can understand the issues they are experiencing from their perspective.
Your intervention will include identification of relevant psychosocial problems, risk assessment, information giving, signposting and referral to specialist and community services.
You will make safeguarding referrals for adults and their children.
You will liaise with clinicians, safeguarding teams and multi-agency partners from specialist and community services to help formulate safe management plans.
You will act as the patients advocate, helping them to achieve the outcomes they desire and, in doing so, will help promote their health, safety and wellbeing.
You will be there to support patients in times of crisis and, in doing so, will gain immense satisfaction from being able to help them access the services they need. For many, this will create positive changes in their lives.
You will also help the clinical team to provide a safe and supportive environment for patients with learning disability and dementia.
You will provide supervision, mentoring and training to less experienced team members. You will contribute to service development, including audit, and will support the process of developing anticipatory care plans for some of our High Intensity Service Users.
You will have:
Experience of working with patients with psychosocial needs in a client-focused health or social care role.
Experience of supporting the identification & reporting of adult and child safeguarding referrals.
A good understanding of mental capacity, confidentiality and consent, when applied to working with vulnerable adults.
A professional appearance, as well as good organisational, interpersonal skills and communication skills.
The ability to manage your own time and work independently, but also be able to work collaboratively within teams.
As a minimum, education to GCSE (or equivalent) standard, with a pass in English and possess an NVQ3 or equivalent level of experience
