The Social Worker MSW will collaborate in the development and implementation of all performance improvement initiatives.
The Social Worker MSW provides or supervises plan of care interventions to include resource and referral assistance, crisis intervention, prevention, education, patient advocacy, bereavement, therapy, and counseling as appropriate. Collaborates with the healthcare team and involves the patient and/or family in the development and implementation of plans. The Social Worker MSW serves as a liaison with the community resources and their staff to facilitate smooth transition and placement of the patients within the assigned service line.
The ideal Social Worker MSW will have knowledge of the principles and methods to restore or enhance social, psychosocial, or bio-psychosocial functioning of individuals, couples, families, groups, organizations and communities. They will have listening and interpersonal skills, verbal and written communication skills and the ability to handle grief.

POSITION SUMMARY:
Under the direct supervision of the Director/Manager of Case Management, the Licensed Social Worker performs discharge planning; and collaborates with the full case management/patient care team for the purpose of optimizing patient care and resource usage. The Licensed Social Worker will evaluate the psychosocial needs of patients and family support systems and will coordinate appropriate discharge plans for identified patient populations. The Licensed Social Worker acts as a liaison between the facility and resources external to the organization. He/she will facilitate timely discharge plans and follow-up on anticipated post-acute interventions identified in the plan of care. The Licensed Social Worker will assist the RN Case Manager in facilitating patient movement across the continuum of care and will identify and track barriers to patient throughput. All functions will be performed in accordance with the standards, practice acts, and licensure in a manner consistent with the mission and goals of the organization.

The Transition Navigator represents the Agency in activities involving professional contacts with patients/families, physicians, hospitals/facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of the Agency’s Hospice services. The Transition Navigator integrates clinical guidelines, protocols and other metrics in development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of care.
The Transition Navigator represents the Agency in external transitional care activities, strategic relationships with health systems, home health, hospitals and other inpatient facilities, physicians and physician groups, and executive level opportunities.
