Community Mental Health Nurse, Band 6
The Intensive Dementia Support Service (IDSS) West would like to invite a Mental Health Nurse RMN to join our innovative team in Taunton, we are a countywide service for people with dementia and cognitive impairment with bases in Yeovil (East) and Taunton West).
The team in the west covers Minehead, Wellington, Taunton, Bridgwater and Burnham and surrounding villages.
You’ll need to have experience of working in the community and an understanding of Older Persons Mental Health Care. You’ll take on a duty worker role so your previous experience in risk assessment and the ability to implement risk management plans will be vital to be able to react to the changing needs of your patients. You’ll also need to have good leadership skills and demonstrated experience of supervising junior staff members.
The IDSS provides a 7-day service between 8am and 8pm with the opportunity to work either:8 hour or 12 hour shifts.
As a team we have received continued recognition for our success in the form of an Award at the Trusts People Awards in 2019 for Quality Improvement and Innovation and we want you to be a part of this too. You too will have a genuine opportunity to help shape, formulate and promote this service, providing you with a unique opportunity to develop your own professional portfolio.
The aim of the team is to be reactive and responsive to treat elderly patients at times of crisis in their usual place of residence, including community hospitals, nursing and residential homes and in acute hospitals. The team work closely with the Community Mental Health Teams and aim to prevent admissions into the Older Persons Mental Health ward and facilitate timely supported discharges from the ward.
We provide intensive support in the form of medication review, occupational assessment and practical nursing approaches and strategies. We also offer ongoing education to the care homes and hospital settings, as well as families and carers, providing high quality patient care, working in a flexible way across the community, day hospital and inpatient boundaries, as required, focussing on the need to support patients and their families/carers.
