We have an exciting opportunity available within Adults Health & Care Reablement Service. We are seeking highly skilled, enthusiastic, and self-motivated Occupational Therapists to join

As a qualified Occupational Therapist, you will be responsible for leading holistic assessments of Reablement needs and planning Reablement interventions to support across a multi-disciplinary team.

There may also be opportunities to work across Hampshire as part of a county wide Reablement Service comprising Occupational Therapy, Sensory Services and CQC Registered Community Response Staff.

We are currently working to develop strong links with our colleagues in Health, integrated intermediate care hubs and hospitals, to support discharge pathways and avoiding unnecessary hospital admissions. You will be instrumental in the success of this service development.

Main duties of the job

Who we are looking for:

  • You will be able to demonstrate effective assessment and treatment planning skills, goal setting and, work in a fast pace community service.
  • You must be able to demonstrate strengths-based approach to provide advice and information for clients to problem solve their own needs and make informed decisions on their Reablement journey.
  • You will have excellent communication, inter-personal and team player skills.
  • There is a responsibility to supervise junior Reablement staff within the team.
  • Previous experience in a Reablement or Rehabilitation setting would be advantageous but not essential.

ABOUT THE TEAM

The post holder will work closely with the established multidisciplinary team and be able to call upon their support as well as that of the current experienced therapists both within neurosciences, and in the wider trust. Within our combined therapy department at UHS Foundation Trust, we employ over 300 Occupational Therapists, Physiotherapists and B2-4 therapy support staff with a huge variety of expertise and specialist skills.

WHAT YOULL DO

The applicant, along with the other band 7 staff, would be required to lead, supervise and develop the neurosciences therapy team, undertake non clinical duties such as ensuring the daily staffing of the unit and appraisals or HR issues with other members of the team. They would be expected to promote therapies, and continue to develop and expand the provision of the service that is provided within the neurosciences department to the highest standard

WHAT WERE LOOKING FOR

We are looking for an experienced, dynamic, driven and motivated therapist who is passionate about all aspects of acute neurosciences and has a healthy understanding of discharge planning and requirements for patients with complex needs. This will help maintain the current high standard of care we deliver and assist in improving the outcomes for our patients.

Current reconfiguration of our staffing structure has allowed us to expand the senior occupational therapy service to offer a greater level of therapy to patients with complex neurological conditions, and then continue this throughout the whole department. The successful applicant would need to have a sound knowledge of acute neurological conditions such as head injury, spinal injury, poly trauma and poly neuropathies. They would also need to be familiar with the management of stroke patients and subsequent pathways, as well as the challenges of complex rehabilitation within the acute setting.

All our therapy staff are required to be able to undertake flexible working over a 7 day period between 7.30am and 8.30pm incorporating weekend shifts, twilight shifts, as required.

Our teams work closely with colleagues in Hampshire County Council and Hampshire Hospitals Foundation Trust to provide the best care and outcomes for our population. Based in the Colden Common office just outside of Winchester in a rural area.

Reablement is a short term, therapist led service aiming to prevent, reduce and delay the need for ongoing services. Reablement supports people to be as independent as possible and includes the provision of information and advice, analysis of needs, provision of equipment and adaptations and targeted assistance from a team of support workers. The reablement team seek to support people to make use of community and personal assets to achieve their goals.

The overall responsibilities of the reablement team are:

To work with customers and carers/families to identify goals and design creative reablement support plans to achieve them.

To connect customers with local resources and facilities.

To operate within legal frameworks and defined budgets for social care.

The team works closely with colleagues from Health, private providers, the voluntary sector and others, to support the occupational needs of customers.

To uphold standards of practice for Occupational Therapists as determined by the regulatory body, the Health and Care Professions Council and the Professional Standards and Code of Ethics and Professional Conduct of the Royal College of Occupational Therapists.

OCCUPATIONAL THERAPIST – BAND 6 NEUROLOGY ROTATIONAL POST (30 hours per week)

An exciting opportunity has arisen to join a strong and dynamic multi-disciplinary team working with patients’ with neurological presentations as part of a rotational post, at the Queen Alexandra Hospital, Portsmouth. The successful candidate will have the opportunity to work closely with all members of the MDT to provide high quality care and treatment. You will alsohave the opportunity to enhance current service delivery and patient experience through both occupational therapy & MDT service developments. We pride ourselves on our close working relationships with all of our MDT members and you will join a very well led and cohesive team of committed Occupational Therapists and technicians.

This post is part of a 12 month rotational scheme that rotates between:

Phoenix Rehabilitation Centre (a 13 bedded neurorehabilitation unit providing specialist multidisciplinary rehabilitation to for adults with complex neurological needs)

Acute neurology OT team (acute assessment and OT intervention within ITU and the acute wards)

Hyperacute and acute stroke unit (34 bedded unit)

Stroke rehab unit (13 bedded unit)

You will need to be a car driver, with full driving licence, or ability to travel between hospital sites for necessary patient visits. The post holder would also need to be willing to work flexibly including weekends and bank holidays on a rota basis.
