The Role

• Plan and manage supervisory staff ensuring maximum efficiency and flexibility to respond to operational needs of the customers.

  • The Trust comprises four Clinical Divisions supported by our Corporate Services Division based at our offices in Prescot, Merseyside. Approximately 8,000 staff serve a population of almost 11 million people.

  • Local Services Division provides specialist inpatient and community Mental Health, Learning Disabilities, Addiction management and acquired Brain Injury services for the people of Liverpool, Sefton and Kirkby, Merseyside.

  • Secure Services Division provides high secure mental health services for the North West of England, the West Midlands and Wales. One of only three trusts in the country that provides these services. The trust also provides medium and low secure services more locally for men and women with mental disorders.

  • Specialist Learning Disability Division provides secure Learning Disability services across Lancashire, Greater Manchester, Cheshire and Merseyside. This is currently delivered from the former Calderstones NHS Trust site in Whalley Lancashire.

  • Community Services Division provides community health services to Liverpool CCG and South Sefton CCG residents, in partnership with North West Boroughs Foundation Trust.

  1. Service Details

  • This post will join 3 Community Geriatricians from LUHFT and 1 from Mersey Care NHS FT. They will be based as part of the Community Services Division. The post will seek to develop, strengthen and provide within the following:

  • A common model across Liverpool and South Sefton in order to strengthen the provision, enable cross cover, peer support, improved governance, support and measurement.

  • The aim of provision should be a minimum of 1 WTE to 150,000 population.

  • Provision of the service from the community and mental health provider will aim to improve integration with relevant other community professionals and dedicated approach to community care.

  • The focus of direct input will be on providing pro-active care to complex elderly patients through overseeing comprehensive geriatric assessment, care planning, assistance with diagnosis and managing complex medical presentations within the following settings:

  1. Patients own homes
  2. Care homes
  3. Intermediate Care Facilities
  4. Frailty Units
  5. Out-Patient clinics

  • Community Geriatricians will work and provide care with other staff under them

  • Community geriatricians will be available to provide daily support and direct supervision to community matrons (both domiciliary and care homes) who will be their primary link into the services of Integrated Care Teams in general.

  • Community Geriatricians will deliver care as part of a MDT approach. Key stakeholders for whom community geriatricians should work alongside and receive mutual support from include:

  1. Community Matrons in particular within Integrated Care Teams
  2. Intermediate care facilities
  3. Care Homes
  4. Old age Psychiatry services
  5. Frailty units and Geriatrics departments within acute trusts
  6. End of Life services
  7. GPs

  • Maintaining links with the respective acute trusts will be secured to ensuring on-going seamless care and positive working relationships. Community geriatricians will have an opportunity to work one day a week on a frailty unit or similar setting.

  • Provide cover when required for physical health reviews for psycho-geriatrics in-patients.

  • Take an active and regular role in providing teaching, expertise in evidence, research and innovation regarding the development of services that deal with frail patients

  • The current model of responsive (less than 2 week) review from GP referrals within out-patient clinics, domiciliary home visits, care home in-reach will be part of each community geriatricians weekly job plan.

  • Community geriatricians will be place-based so as to ensure a high level of relational coordination with key stakeholders (GPs, CM, falls teams, care homes, Integrated Care Homes, psycho-geriatrician services etc).

  • The post will require active engagement with GPs and their practice staff in developing the service, understanding, cascading of knowledge.

This is an exciting new post that has been created to support further development in the system wide response to improving community care, frailty care and reablement services across Liverpool. The key aim of this post is to provide specialist Consultant Community Geriatrician support of patients and clinicians caring for patients with clinical markers of frailty and frailty syndromes.

You will implement the end to end manufacturing plan in order to achieve the sites' productivity objectives, contributing to business planning that defines the manufacturing budget.  You will monitor and take corrective action to ensure that labour and materials are being utilised in cost efficient processes which delivers product to manufacturing food safety and customer standards.

As Operations Manager you will work across a number of different areas, consulting and advising relevant departments to ensure the effective and efficient use of all new capital investment projects, highlighting any bottleneck processes and taking mitigating action that will deliver labour/waste.

You'll deputise for the General Manager and will support your team to ensure they strive to the highest of standards at all times, providing inspirational leadership and direction to the operational community to ensure that people are kept safe, engaged, focused and always delivering to their potential.

You'll have a good standard of education either through full time further education or gained as a work based apprentice.

An exciting opportunity as an Assistant Service Manager has arisen within the Urgent Care Division.

The Assistant Service Manager provides essential support to the Service Manager in the operational delivery of a portfolio of clinical specialties, and is accountable to the Directorate Manager.

With full line management responsibility for the Medical Secretary Team Leaders within the specialty administration teams, the Assistant Service Manager is responsible for ensuring a smooth and effective, high quality administrative service is provided to patients, clinicians and Trust colleagues.

The post holder is responsible for ensuring that day to day clinical activity runs as effectively and efficiently as possible. The ability to identify activity variances and pressures is essential, as is the ability to problem-solve and provide solutions in order to improve specialty performance against the required targets, including 18/26 week Referral to Treatment and Cancer 14/31/62 day standards.

Experience of target management and staff line management are essential for this post, and experience in healthcare management is desirable.
