Primary Care Network Clinical Pharmacist

East Suffolk Primary Care Network is a large innovative partnership of 16 practices providing primary healthcare to more than 150,000 patients. The collaboration sees doctors and other practice staff sharing resources and working together to ensure patients continue to receive high quality healthcare.

We are looking to add to our large team of pharmacists and technicians based in Stowmarket. You will work as part of a team to develop and deliver medicines optimisation services across our Primary Care Network (PCN). PCNs are an exciting new initiative and Suffolk Primary Care is at the forefront of innovation and collaboration.

This is a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care. We offer flexible roles dependent on the hours you are available for work and your level of experience.

You should be able to demonstrate excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews.

You will work as part of a multidisciplinary team to develop and deliver the PCN DES (Direct Enhanced Service) and run processes for repeat prescription reauthorisation, management of medicines on transfer of care and systems for safer prescribing.

We offer a significant training pathway through the CPPE and we will support you to develop, working in an environment that nurtures and develops quality.

We are looking for independent prescribers or those wishing to work toward the qualification. Experience in General Practice would be an advantage but is not essential if you are clinically up-to-date and understand the process of a medication review within a Primary Care setting.

Medacy is looking to recruit clinical pharmacists to support practice working remotely on Vision.

We are looking for 8 hours of support per week, ideally as two four-hour blocks but 2 four-hour sessions in one day will be considered.

The role will involve playing a full and active role as part of the practice multidisciplinary team.

The ideal candidate for the role will have:

  • Independent Presciber Status
  • Previous GP Practice experience
  • At least 3 years post-graduate experience
  • Excellent Clinical knowledge
  • The commitment to continue to learn and develop.
  • The confidence to work as part of a busy GP practice
  • The ability to adapt to convey information to patients and fellow healthcare professionals.
