        1. Clinical Function

        The post holder shall:

        1.1 assist the patient in all aspects of personal care, following the individualised care plans prescribed, maintaining privacy and dignity

        1.2 promote open and sensitive dialogue in order to support the patient and family

        1.3 assist in the establishment of caring relationships with patients and their families, including those with challenging behaviours, those with communication difficulties and those whose first language is not English, using a diverse, appropriate and intuitive range of compassionate communication skills.

        1.4 communicate and liaise with the key worker, reporting any change in the patients condition

        1.5 maintain confidentiality at all times.

        1.6 practice within Garden House Hospice policies and procedures, for the care of patients in their own homes

        2. Health and Safety Function

        The post holder shall:

        2.1 take all measures to maintain the safety and well being of patients, relatives and staff in accordance with the Health and Safety Act and Hospice Health and Safety Policy.

        2.2 become familiar with procedures for dealing with incidents such as accidents or fire and report and escalate any accidents, incidents to Sister/Senior Sister.

        2.3 attend fire lectures annually and take part in maintaining fire safety within the building.

        3.4 develop competency in designated clinical skills.

        3.5 help promote hospice and palliative care philosophy inside and outside the Hospice

        6.0 Additional Responsibilities

        6.1 There is a requirement to comply with all the hospice policies, procedures and guidelines, including those relating to Health & Safety, confidentiality and the Data Protection Act, as required by Information Governance.

        6.2 Each member of the hospice team has a responsibility to contribute to the clinical governance programme.

        6.3 Each team member to take appropriate action to maintain the highest level of infection prevention and control.

        8.0 Data Security & Protection All Hospice staff are required to comply with legislation pertaining to Data Security and Protection which incorporates Information Governance and General Data Protection Regulations. Staff are expected to familiarise themselves with the content of the Data Security and Protection policy and comply with these requirements. Any breach of this policy must be raised to the Data Protection and Security responsible officer following the correct reporting procedure outlined in the policy.

        9.0 Enablement To support patients in a way that gives them choice, autonomy, maximises their function and wellbeing and enables them to live as independently as possible within the limitations of illness wherever Garden House Hospice Care support is given.
