We are looking for dynamic and motivated Healthcare Assistants with a strong Rehabilitation and Reablement ethos to join our friendly and supportive team in the Skylark Rehabilitation Unit located in The Meadows, Yate.

There are 30 beds in the new unit and we will be providing intensive rehabilitation for people who are unable to safely return directly home following a hospital stay due to on-going rehabilitation and care needs. We will also support those people who need more complex discharge planning to enable them to return home. We are passionate about delivering high quality care to the people we support while helping them to reach their full functional potential and optimise their well-being.

We actively promote a multi-disciplinary model of care and our nursing, therapy and support teams work closely with our social care and community teams, to make sure those we support have an exceptional experience. We also work with our GPs with Specialist Interest and other medical colleagues to ensure we provide care of the highest standard.

The successful candidates will have a real passion for their work, with excellent communication skills, a positive attitude, and respect for those we support, their carers and families as well as our colleagues in the wider Multi-Disciplinary Team. You will join a team of experienced nurses, therapists and support staff to provide compassionate care, and will work with us to create an environment providing high quality rehabilitation and reablement.

This is an exciting opportunity to join our IBD Nurse Specialist nursing team in a band 6 development post, moving to band 7 after completing relevant further training and competencies.

We are looking to welcome a highly motivated, confident and enthusiastic nurse who will be an integral part of our supportive IBD nursing service. The successful applicant will work closely with the Lead IBD Nurse Specialist and MDT to provide excellent, specialised nursing care and support for patients with IBD in inpatient and outpatient clinic settings.

You will be welcomed into a supportive IBD nurse service where we will support you to develop expert knowledge and skills in order for you to provide excellent, evidence-based professional advice.

Duties will include managing nurse-led face to face and virtual outpatient and biologic clinics, in-patient clinical reviews, and providing expert clinical advice and information via the direct-access patient telephone and email service.

You will provide comprehensive clinical assessment, advice, education and support to patients to promote self-care in IBD disease and medications management. You will implement early investigations, changes in treatment/medication and facilitate rapid access to secondary care services as the primary point of contact for patients, relatives, carers and clinicians, in relation to the management of IBD.

The successful applicant will need to have Gastroenterology or related experience, with excellent communication and IT skills. You should be confident, enthusiastic, self-motivated and with the ability to work under your own initiative, in collaboration with the MDT team.
