An exciting and challenging opportunity has arisen for a Practice Nurse to join our friendly practice team. We are looking for a highly motivated professional who can demonstrate high standards of clinical excellence.
Experience working in a general practice environment would be an advantage but is not essential.
We have 9,200 patients (approx.) and work from purpose built premises using the SystmOne computer system. We have a consistent high QoF achievement and are CQC rated as Good in all areas (most recent visit April 2018).
You would be part of a friendly, experienced practice team with a full complement of GP’s, nursing team and administrative support staff. Special interest & personal development would be supported and practice culture promotes excellent work / life balance. The post offers an excellent salary and 6 weeks holiday (pro rata).

The successful candidate will work in all areas of the department including 24 hour emergency on call at both the North Tees and the Hartlepool site and at regional satellite sites as part of the Bowelscope programme

The successful applicant will work within a multidisciplinary team and across clinical boundaries in support of patient care. Good communication is essential as you will be expected to communicate effectively with patients their relatives and carers and other professionals working in the trust.

We have an exciting opportunity in our renal unit in Stockton. The clinic in Stockton works with South Tees Hospitals NHS Foundation Trust and currently has 15 beds. The clinic is open 24 hours on Monday, Wednesday and Friday and on Tuesday, Thursday, Saturday, they are open until 18:30. There is free parking and the clinic also has good bus/public transport links.
We are looking for a NMC registered nurse who wants to make a difference in our patient’s lives. You will provide patient care according to Diaverum procedures and in order to increase the quality of life for our patients. A comprehensive professional training programme will be provided to you, including on-the-job training, e-learning and access to our in-house education programme ‘d Academy’.
RESPONISIBILITIES:
• Take ownership of care programmes for renal patients including the initiation and termination of dialysis, whilst providing stable clinical care, emotional support and independence for patients
• Follow the tailored care plans to administer medications, fluid therapy or other appropriate treatments as per Consultant instructions and ensuring clinical activity is appropriately documented in the patient medical record
• Review pre and post haemodialysis care with patient
• Discuss health concerns with patient; review patient concerns with the multi-disciplinary team as necessary
• Provide on going patient education
• Prepare and monitor dialysis machines and systems inline with Diaverum processes
• Monitor dialysis water treatment system for use according to the established procedures
• Act as a named staff nurse for a group of assigned patients ensuring ‘great care‘ is provided at all times
• Proactively participate in quality and service improvement programmes
REQUIREMENTS:
• NMC Registered Nurse
• Awareness and Understanding of NMC code of professional conduct
• Be able to work autonomously and within a multi-disciplinary team
• At least one year’s experience in general nursing (Haemodialysis would be an advantage but not essential) - full training is provided.

Stockton Quality Control Laboratory is the regional NHS Pharmaceutical Quality Control Laboratory for the North East, North Cumbria and also supplies services to NHS Trusts in Yorkshire and many other trusts around the UK. The laboratory supplies a wide range of quality assurance services to pharmacy departments, hospital estates departments, sterile services departments and other units with clean room facilities. The service also has customers within the private sector and supplies services such as contract analysis to these clients.
The laboratory is based at the University Hospital of North Tees which is part of North Tees and Hartlepool NHS Foundation Trust. Stockton Quality Control Laboratory is part of the Pharmacy Department which is in the Clinical Services Directorate.
We require a motivated team player with attention to detail who under supervision will be responsible for the reading of routine microbiological test samples from pharmaceutical production environments. This role may also include the handling and maintaining of supplies, maintenance of equipment, ensuring that the laboratory is clean and tidy and preparing goods for delivery to laboratory users and recording goods and tests in IT systems.
Please refer to the job description and person specification for a more detailed outline of duties. Experience of working in a laboratory or healthcare environment would be an advantage.
The successful applicant may also be expected to be part of the Saturday morning rota. The post holder needs to be reliable and adaptable to the needs of the service as required.
