An exciting opportunity has arisen for an experienced community nurse to join our dynamic Care Coordination Centre base in Burton-upon-Trent. This Band 6 role will work within this Centre to co-ordinate and ensure provision of effective processing of all referrals and messages received relating to the community services provided in East Staffordshire. The successful applicant will have experience and knowledge of providing physical nursing care.

The Care Coordination Centre is an established single point of contact for service users and their carers as well as healthcare and social professionals. The Centre is a 7 day service which excludes bank holidays and the Band 6 Sisters work Monday to Friday. We process all referrals for the community services that we provide over East Staffordshire.

Correct triage of patients ensures that the patient is seen by the correct service, in a timely manner, whilst focusing on the self-care model, health promotion and independence where possible. You will be a pivotal part of the Care Coordination Centre team, providing comprehensive triage.

We work closely with partners particularly Primary care colleagues; the local secondary care establishments; allied health professionals; specialist nurses; community unplanned care teams; community hospitals; emergency services; social care and mental health services, charities and voluntary services.

IT skills are a must, and you will be clinically competent to be able to provide evidence based care and advice. You will have effective leadership skills and be able to co-ordinate the working day, as well as complete off duty, appraisals and supervisions for your team. You will be compassionate, organised and be solution focused. You will work as part of the multi-disciplinary team, using your excellent communication skills to network with others, demonstrating innovative ideas, to improve patient care. You will be a registered nurse and have experience of providing physical (general) nursing. A full induction package will be provided upon appointment.

What will you be doing?

You’ll liaise with architects, planning consultants and planners on development schemes as required. You'll assess possible development projects for financial return, customer demand, likely take-up and feasibility. You'll undertake detailed appraisals of major schemes.

Constantly looking for opportunities to develop the business which will provide benefit for customers and the general public and as a consequence provide profitable growth.

Help determine the optimum technical/engineering solutions.

Develop an overview of existing products, operations and processes and put forward suggestions for improvement.

 

What experience and qualifications do you need?

Business Designer

  • Diving deep into data, mapping processes and problems.
