We are looking to recruit an experienced Rehabilitation Assistant looking for a new challenge and would like to join our expanding Rapid Response Team in a role that will provide short term rehabilitation interventions.

The successful applicants will be part of a Multidisciplinary team, passionate about admission prevention.

The successful applicants will work alongside Therapist’s and members of the MDT to support the daily work of the rapid response team. As part of the team you will be working with patients who require early/urgent therapy intervention to remain independent within their own home environment and prevent unnecessary hospital admission.

Rapid Response is a county wide team working as part of the new ‘Somerset Intermediate Care Service’, working alongside the 4 Neighbourhood’s within Somerset. Therapists will be based from either West Mendip/Bridgwater/Dene Barton or South Petherton Community Hospitals, working within a neighbourhood but due to capacity, and staffing issues such as sickness, leave and vacancies a cross country cover will be expected.

Main aims of the service is to

  • Prevent unnecessary hospital admission
  • Keep service user to their optimal functional level in their own home
  • Reduce the need for ongoing care and potentially reduce referral on to Discharge to assess service buy using the enablement approach.
  • Provide support to enable quick turn around of patients from Hospital to prevent longer hospital stay than is needed

Other aspects of this role is to actively link work along side the JETT team(MPH) and Rapid turn around team (YDH) and equivalents In RUH and WGH, enhancing the conversation and support positive risk taking to discharge patient back to their own environment

To support transition from the service another key aspect of the role will be to have understanding of other community services within the county and build/form relationships with Community teams/Voluntary Sector and Adult Social Care team’s around the county to allow a seamless transition.

Hours of work for this post is between 8 am and 8 pm, 7 days a week. You will be expected to work in shifts during the week alongside to ensure the full day is supported from a therapy point of view.

Their will be a expectation to work along side the Therapy team to provide training regarding enablement/rehabilitation approach.

Line management will be provided by newly created Band 6/5 RR therapist’s.

We have a great opportunity for you to join our new and developing community based admission avoidance service in Somerset.

The key aim of the new service is to prevent unnecessary admissions to hospital, providing a rapid short term intervention to adults in their own homes.

We are looking for dynamic individuals who work well in a team and have a flexible ‘can do’ attitude to providing care and support to patients in their own home.

Previous community experience is desired, but not essential as the teams will give you the support and guidance you need to succeed in the role. We will ensure we support you to develop your skills through on the job training.

You'll be required to work shifts over 7 days a week between the hours of 08:00 and 23:00, and 21:45hrs and 08:15hrs for night shifts. Flexible working requests can be considered and will be discussed at interview.

The service operates across Somerset targeting key areas with high numbers of hospital admissions and has bases in Taunton/Bridgwater, South Somerset area, and West Mendip. You will expected to work either in pairs or individually and be must prepared to do home care visits alone depending on patient needs. A clinical Supervisor also oversees each shift, providing support as and when required.

As this is a community based position, a driving license with access to your own vehicle and business insurance is essential. Hours and shifts are offered on a full or part time basis.

We are looking for Support Workers who are confident, flexible and competent with good communication skills.

We are a specialist service for people between the age of 14-65 who are experiencing, or at high risk of developing, their first episode of Psychosis.

You’ll have the opportunity to work autonomously with the support of a strong team of other health care professionals. The role will involve providing assessments, care coordination, outreach and offering support to family members/carers, through education and liaison for people with first episode psychosis in the locality in order to reduce the duration of untreated psychosis.

We are delighted to have recently received additional investment to improve our services and have also been graded as a top performing team by the NCAP Early Intervention in Psychosis Spotlight audit, making it a great team to be a part of.

We are able to offer training within the role such as a 10 day course in Family Intervention in conjunction with Exeter University, as well as regular supervision and support from your manager.

About you

Experience with Learning Disabilities or Mental Health support for adults or children, either through employment or your own personal experiences is required for this role. Recognised care certificates such as NVQ or CQF are desirable but not essential as we can offer training from day one and fantastic career pathways to develop you to gain these qualifications.

Our main therapy model is Dialectical Behavioural Therapy and there potential for you to become involved in this programme and access further specialist training. You will also take part in a variety of therapeutic activities with young people, related to their care plans.

Are you an experience Rehabilitation Assistant who has a passion for patients staying at home, working in a unplanned way and avoiding Hospital Admission where possible?

We are looking to recruit an experienced Rehabilitation Assistant looking for a new challenge and would like to join our expanding Rapid Response Team in a role that will provide short term rehabilitation interventions.

The successful applicants will be part of a Multidisciplinary team, passionate about admission prevention.

The successful applicants will work alongside Therapist’s and members of the MDT to support the daily work of the rapid response team. As part of the team you will be working with patients who require early/urgent therapy intervention to remain independent within their own home environment and prevent unnecessary hospital admission.

Rapid Response is a county wide team working as part of the new ‘Somerset Intermediate Care Service’, working alongside the 4 Neighbourhood’s within Somerset. Therapists will be based from either West Mendip/Bridgwater/Dene Barton or South Petherton Community Hospitals, working within a neighbourhood but due to capacity, and staffing issues such as sickness, leave and vacancies a cross country cover will be expected.

Main aims of the service is to

  • Prevent unnecessary hospital admission
  • Keep service user to their optimal functional level in their own home
  • Reduce the need for ongoing care and potentially reduce referral on to Discharge to assess service buy using the enablement approach.
  • Provide support to enable quick turn around of patients from Hospital to prevent longer hospital stay than is needed

Other aspects of this role is to actively link work along side the JETT team(MPH) and Rapid turn around team (YDH) and equivalents In RUH and WGH, enhancing the conversation and support positive risk taking to discharge patient back to their own environment

To support transition from the service another key aspect of the role will be to have understanding of other community services within the county and build/form relationships with Community teams/Voluntary Sector and Adult Social Care team’s around the county to allow a seamless transition.

Hours of work for this post is between 8 am and 8 pm, 7 days a week. You will be expected to work in shifts during the week alongside to ensure the full day is supported from a therapy point of view.

Their will be a expectation to work along side the Therapy team to provide training regarding enablement/rehabilitation approach.

Line management will be provided by newly created Band 6/5 RR therapist’s.
