Cheeki Monkeys are recruiting event organisers for a varied role within our already successful team.

 

As a Cheeki Monkeys event organiser, your tasks will involve:

 

  • Sourcing venues to hold our events
  • Uploading upcoming event dates to free event listing websites and parent forums
  • Promoting our Nearly New Baby Sales via Social Media platforms
  • Building relationships with family orientated business
  • Overseeing event days; venue setup/directing stallholders to their allocated pitches etc.

Blackpool Central Primary Care Network are looking to recruit a cancer care co-ordinator to our team. The successful candidate will work with the 5 GP practices within the network to deliver effective personalised care to patients at any stage of their cancer journey.

The care co-ordinator will work directly to support patients, as well as working across the practices and patient population to help deliver the requirements of the PCN enhanced service for cancer care.

This is an exciting new role with scope to develop and deliver service improvements, working with the GP practices and wider community health teams.

The Blackpool Central PCN was formed in July 2019 in response to NHS legislation and consists of the following practices:

The five practices have a dedicated community team attached to them which contain health and well-being workers; district nurses and a full nursing home team, plus others.

We are in the exciting position of adding new roles to work across the five practices and compliment the current surgery teams, to give our patients a patient-centred care approach.

Personalised Care:

The applicant must have the ability to work within the framework of The Personalised Care Model as set out by the Personalised Care Institute.

Key Responsibilities:

The applicant will be working with a cohort of patients who have either been diagnosed with cancer, or in the process of being investigated for suspected cancer.

The applicant must be able to do the following

1. utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care;

2. support patients to utilise decision aids in preparation for a shared decision-making conversation;

3. holistically bring together all of a persons identified care and support needs, and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person;

4. help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care;

5. support people to take up training and employment, and to access appropriate benefits where eligible;

6. support people to understand their level of knowledge, skills and confidence (their Activation level) when engaging with their health and wellbeing, including through the use of the Patient Activation Measure;

7. assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level;

8. explore and assist people to access personal health budgets where appropriate;

9. provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals;

10. support the coordination and delivery of MDTs within the PCN.

Key Wider Responsibilities:

The applicant will

1. work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN;

2. raise awareness within the PCN of shared-decision making and decision support tools;

3. raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations

Specific Responsibilities Relating to the PCN DES:

The applicant will be expected to support and work alongside the PCN practices to fulfil following requirements of the PCN DES. The applicant would be a key person in co-ordinating this work across the PCN.

The DES states that from 1 October 2020, a PCN is required to:

1. Review referral practice for suspected cancers, including recurrent cancers. To fulfil this requirement, a PCN must:

a. review the quality of the PCNs Core Network Practices referrals for suspected cancer, against the recommendations of NICE Guideline 1252 and make use of:

i. clinical decision support tools;

ii. practice-level data to explore local patterns in presentation and diagnosis of cancer;

iii. where available the Rapid Diagnostic Centre pathway for people with serious but non-specific symptoms

b. build on current practice to ensure a consistent approach to monitoring patients who have been referred urgently with suspected cancer or for further investigations to exclude the possibility of cancer (safety netting), in line with NICE Guideline 12;

c. ensure that all patients are signposted to or receive information on their referral including why they are being referred, the importance of attending appointments and where they can access further support

2. contribute to improving local uptake of National Cancer Screening Programmes. To fulfil this requirement, a PCN must:

a. work with local system partners including the Public Health Commissioning team and Cancer Alliance to agree the PCNs contribution to local efforts to improve uptake which should build on any existing actions across the PCNs Core Network Practices and must include at least one specific action to engage with a group with low-participation locally;

b. provide the contribution agreed within timescales agreed with local system partners;

3. establish a community of practice between practice-level clinical staff to support delivery of the requirements set out in sections7.4.1.ato 7.4.1.b.A PCN must, through the community of practice:

a. conduct peer to peer learning events that look at data and trends in diagnosis across the PCN, including cases where patients presented repeatedly before referral and late diagnoses;

b. engage with local system partners, including Patient Participation Groups, secondary care, the relevant Cancer Alliance,and Public Health Commissioning teams

    JOB TITLE: EXPERIENCED RECEPTIONIST

    REPORTS TO: Reception Supervisor

    HOURS: Full or Part Time Considered

    Main Duties

    • Ensure an effective and efficient reception service is provided to patients and any other visitors to the Practice
    • Deal with all general enquiries, explain procedures and make new and follow-up appointments. Arrange home visits ensuring careful recording of all relevant details for the doctor. Deal with patient bookings and enquiries in a friendly and courteous manner both face to face and over the telephone.
    • Explain Practice arrangements and formal requirements to new patients and those seeking temporary cover and ensure procedures are completed. Ensure that all new patients are registered onto the computer system promptly and accurately.
    • Scanning accurate scanning and attaching of correspondence to patient records.
    • Ensure accurate and timely recording of Data with attention to the use of Snomed Codes.
    • Action prescriptions in an accurate and timely manner including repeat/controlled and acute prescriptions as per practice prescribing protocols. Audit and remove prescriptions not collected by patients.
    • Results - to accurately process results and allocate them to the appropriate clinician for review and action.
    • Act on clinicians requests for further information/patient contact information etc. (including computerised practice notes), ensuring completion of all associated paperwork.

    Other Duties:-

    • Open premises at the start of the day when first to arrive, de-activate alarm and make all necessary preparations to receive patients.
    • When last to leave at the end of the day, ensure that the building is totally secured, internal lights are off, and the alarm activated.

    You will be working in a small team and assisting with all aspects of customer service, product preparation and order fulfillment. We offer our extensive catalogue of premium supplements to both domestic and international customers including re-sellers.

    Key Responsibilities will include, but are not limited to:

    • Provide first class customer service when handling customer queries, questions and requests via email and telephone.
    • Efficiently and accurately label, pick and pack customer orders ready for dispatch
    • Product preparation including labeling, heat sealing packages and bubble wrapping fragile items
    • Assist with the preparation of shipments to Amazon fulfillment centers
    • General duties including processing customer returns and resolving any order issues
    • Work closely with senior members of the team to ensure the efficient day to day running of the business

    Requirements: 

    • An interest in nutrition and the supplements industry
    • An enthusiastic desire to learn with a can-do attitude
    • Be flexible and adaptable to the needs of the business
